SKILLED NURSES/CAREGIVERS
Our Registered Nurses are committed to provide quality care in all our nursing services. They are highly skilled in specialized nursing and their interpersonal skills are practiced through effective communication with patients, families and other members of the health care community. Our Home Health Aides work directly under the supervision of Registered Nurses, Physical and Speech Therapists. They report all data to the appropriate supervisor while delivering care to the patient in the home environment.
REHABILITATION
Our experienced therapists regularly coordinate and communicate with the patient, physician and other team members. They perform evaluations and assessments, set a plan of care, teach and educate clients/caregivers, and families. Our highly qualified therapists provide services in the areas of cardiopulmonary, geriatrics, neurological disorders and musculoskeletal problems. They strive to achieve the functional mobility and independency of their clients to ensure their safety.
DIABETES/NUTRITION
Diabetes can be monitored and self managed effectively in the patient’s environment with proper education and teaching by our professional staff. Nutrition can be individualized through a plan for every patient referred for consultation and services. Our nutritionists are skilled in developing special diets, and are experts in counseling patients in menu planning and nutritional care.
MEDICAL SOCIAL WORKERS
Medical Social Workers help to resolve social, emotional and financial issues that might be expected or considered to hinder treatment of the stated rehabilitation potential.
CASE MANAGEMENT
Case Management, a modern approach to health care, is a venture to save cost and hospital stays by caring for the patient at home with appropriate support services. Common patient needs may include: Acute Illness, Intensive Treatments, Intensive Rehabilitative Therapy, and Individual Prescribed Treatment.
